This is an SPF 25 lip balm from Rohto mentholatum, can I still use it even tho I left it in the car by mistake ? It’s hot as hell here , and the balm itself seems a bit melty.

Uh it’s probably degraded a little from the heat but still fine to use. Especially if it was in a bag or in your glove box. It would probably be worse if it was exposed to light for an extended period of time because the spf will degrade. The balm will probably be less melty after it sits in room temperature for a while.
My uneducated assumption is it gets hot af in Japan in the summer and not all delivery trucks have air conditioning for their cargo…so if these products can survive getting shipped under those conditions it probably survived your car???
Always rely on your senses though, if it looks discoloured or tastes or feels off, just toss it out :/
